Infertility In Men And Women (PART 2)

MALE FACTOR INFERTILITY

Infertility affects both men and women. Factors responsible for infertility in men are called Male Factors of infertility and they cause 35% of all cases of infertility.

SOME CAUSES OF MALE INFERTILITY

Causes of male infertility are mostly testicular and can be divided into:

A)Pre Testicular

B)Testicular

C)Post Testicular

A)PRE-TESTICULAR FACTORS OF MALE INFERTILITY

These can be congenital or acquired diseases of the hypothalamus, pituitary or peripheral organs that alter the hypothalamic-pituitary axis. Such diseases include:

1)Idiopathic hypogonadotrophic hypogonadism

2)Prolactinomas

3)Gonadotropin Deficiencies

4)Cushing Syndrome

B)TESTICULAR FACTORS OF MALE INFERTILITY

These factors can be genetic or non-genetic.

Klinefelter’s Syndrome is the most common chromosomal (genetic) cause of male infertility. It results in primary testicular failure.

Non-genetic causes of male infertility include:

1)drugs

2)radiation

3)infections

4)trauma

5)varicoceles

Ageing also affects fertility in men. As a man ages, testosterone levels decrease and gonadotropin levels increase, sperm concentration and semen volume change and libido decreases.

In addition, the incidence of having a baby with a birth defect increases with increasing age

These are factors that do not allow the normal transport of sperm through the ductal system. They can be congenital or acquired.

Men who are exposed to DES (diethylstilbestrol) in-utero may have ductal obstruction. Congenital bilateral absence of the vas deferens is seen in men with cystic fibrosis.

Infections, trauma and surgical procedures may cause ductal blockage.

Infertility is failure to conceive after 12 months of having regular unprotected sexual intercourse.

This condition affects approximately 10 – 15% of couples of reproductive age.

It is important to note that infertility is found in both men and women
